Cancellation Policy

Registrations must be made no later than August 15, 2018. With written cancellation notice, received by July 18, 2018, you will receive a full refund less a $50 processing fee. Cancellations after July 18, 2018 are nonrefundable. Notices of cancellation must be faxed to the MDLA office at 651-290-2266. No-shows will not receive a refund.

Admission to all MDLA events requires advance registration either by mail, online, or in person. Individuals will not be admitted to an event without payment. Failure to attend an event does not constitute a cancellation of event registration and you will be charged for registering.

Per PCI Compliance standards, the pdf form will not be accepted via email. Please mail or fax submissions.
