Registration fee include access to all sessions, welcome reception, continental breakfasts, lunch at the Annual Meeting, refreshment breaks, course materials, and certification credits (8 credits have been applied for).

Cancellation Policy
Registrations must be made no later than August 4, 2016. With written cancellation notice, received by July 19, 2016, you will receive a full refund less a $50 processing fee. Cancellations after July 19, 2016 are nonrefundable. Notices of cancellation must be faxed to the MDLA office at 651-290-2266. No-shows will not receive a refund.
Per PCI Compliance standards, the pdf form will not be accepted via email. Please mail or fax submissions.
This event may be recorded and/or photographed. Completion of the registration form acknowledges consent to be filmed or photographed.
Admission to all MDLA events requires advance registration either by mail, online, or in person. Individuals will not be admitted to an event without payment. Failure to attend an event does not constitute a cancellation of event registration and you will be charged for registering.
